- The assessment is to ascertain whether the child is dyslexic. This assessor does not assess for dyscalculia, dyspraxia, ADHD or ASD. If any of these areas of co-occurring or ASD difficulties arise during the assessment, the child will be referred on either to an appropriate specialist or to an appropriate specialist via their GP.
- Prior to the assessment the child must have had an eye test in the last 6 months.
If the child has been struggling with their hearing, a hearing test may be recommended.
- To book an assessment: A Booking Form is be completed and returned.
An invoice and BACS details will be sent to the email address on the Booking Form so a £65 deposit can be paid.
- Once the Booking Form has been received, a questionnaire will be sent to both the child’s parent(s) and teacher/tutor for completion.
- The questionnaires need to be returned 1 week prior to the assessment.
- The remaining payment of £400 (made by BACS or cash) must be made at the time of the appointment. It can, however, as with the deposit, be made in advance.
- The report will not be released until full payment is made.
- The report will be sent within 15 working days of the appointment.
- This assessor has the right to:
-Not accept a referral if it appears to be outside her area of expertise.
-Terminate an assessment if the child fails to co-operate with the assessment which would compromise the test results.
-Not to release the report until full payment is made.
Data Protection.
The report will be processed in accordance with the Data Protection Act and the General Data Protection Regulation (2018). The information will only be shared with third parties with your consent unless there is a legal requirement to do so. Your information will be securely stored for 7 years, after which it will be disposed of in a secure manner. The report will be securely stored and encrypted until the assessed is 25years old, after which it will be securely destroyed.
