What information I collect, use, and why.
I collect or use the following information for a full diagnostic dyslexia assessment:
- Names and contact details.
- Background information relating to the pupil’s difficulties with learning.
- Health information (including dietary requirements, allergies and health conditions).
I also collect or use the following information for a full diagnostic dyslexia assessment:
- Racial or ethnic origin
I collect or use the following information for 1 to 1 tutoring:
- Names and contact details.
- Background information relating to the pupil’s difficulties with learning.
- Health information (including dietary requirements, allergies and health conditions).
I also collect or use the following information for 1 to 1 tutoring:
- Racial or ethnic origin.

My lawful base for collecting or using personal information for a diagnostic dyslexia assessment or 1 to 1 tuition is:
Where I get personal information from:
- Background questionnaires about the pupil’s development, learning and difficulties with learning from both their parents and their teacher(s).
- People directly.
How long I keep information.
The background information questionnaires and the test result sheets from a full diagnostic assessment will be kept for 7 years from the date of the assessment and then securely destroyed.
The report of the pupil’s full diagnostic assessment for dyslexia will be kept securely until the pupil is 25years old. It will then be securely destroyed.
The work of the pupil from a 1 to 1 session will be kept for 7 years from the end of that tuition before being securely destroyed.
The information collected for effective 1 to 1 tuition to take place will be kept for 7 years before being securely destroyed.
Any paper information kept will be held in a locked secure cabinet.
All completed questionnaires and reports are encrypted and stored securely.
Who I may share information with.
- When there is a need for a referral letter on to another professional – a GP, Occupational Therapist, Physiotherapist, or Behavioural Optometrist – the relevant only information concerning this referral will be shared with the appropriate professional.
- The pupil’s school if the school has authorised the diagnostic report to be done.
- The pupil’s school if you have authorised the report independently and give permission for this report to be shared with them.
- The pupil’s university or college or employer if you have given permission.
Your data protection rights.
Under data protection law, you have rights including:
Your right of access – You have the right to ask us for copies of your personal data.
Your right to rectification – You have the right to ask us to rectify personal data you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.
Your right to erasure – You have the right to ask us to erase your personal data in certain circumstances.
Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal data in certain circumstances.
Your right to object to processing – You have the right to object to the processing of your personal data in certain circumstances.
Your right to data portability – You have the right to ask that we transfer the personal data you gave us to another organisation, or to you, in certain circumstances.
Your right to withdraw consent – When we use consent as our lawful basis you have the right to withdraw your consent.
You don’t usually need to pay a fee to exercise your rights. If you make a request, we have one calendar month to respond to you.
